STATE UNIVERSITY OF IOWA ADVANCE PAYMENT STATEMENT Attach check or money order payable to the State University of Iowa to this form and mail to : Director of Admissions State University of Iowa Iowa City Iowa YEAR, SESSION, Payment Due by: By: ADVANCE PAYMENT, AMOUNT $50.00 TREASURER'S OFFICE COPY ACCT: P129 CLASS 5200 When submitting your advance payment, please enclose the Advance Payment Statement (all three parts) you received with your admission letter. We will not send you a receipt for your deposit. Your canceled check or money order stub will be your record of payment. Refund Dates In the event an application for admission and/or dormitory accommodations is not withdrawn in writing prior to August 1, the advance payment of $50.00 is forfeited; If the dormitory application is canceled prior to the refund date and the student plans to enroll, the $50.00 advance payment will be held in the student's University account and credited toward payment of fees. If the student fails to withdraw his dormitory application prior to the refund date and enrolls at the University, the dormitory contract is binding and the student will be expected to reside in the dormitory according to terms of the contract. MEDICAL EXAMINATION Each student admitted to the State University of Iowa for the first time is required to submit a medical examination report to the University Student Health Service before his initial University registration. Thereafter, he is eligible to report to Student Health Service for medical care. Please contact your family physician and make an appointment for your medical examination. After this is completed ask your physician to mail the medical examination forms to the University Student Health Service in self-addressed envelope. RESERVE OFFICERS TRAINING CORPS There are two branches of the Reserve Officers Training Corps (ROTC) at the State University of Iowa - Army and Air Force. Every male freshman student who is physically fit must complete one year of basic ROTC with either the Army of the Air Force to be eligible for graduation. Exceptions to this requirement are listed on the following page. 13
