EXHIBIT B1 Visiting Nurse Association Discontinuance of the $25,000 support from the City of Iowa City to the VNA would mean the release of nursing staff, resulting in a return to a nursing program restricted primarily to bedside care. There could then be little program, if any, for the control of: 1. Tuberculosis and maintenace of the TB Registry. 2. Enteric diseases (follow-up and control). It would definitely curtail the immunization program, especially the Well Child Clinic where 700 children were served in 1970. THis would be decreased by about one half to two thirds. Staff would be insufficient for health promotion (including the visits to post partums, which in 1970 amounted to 458 calls). An example of tax dollars which may have been freed for other purposes due to the services of the VNA include: This 69 year old man with diabetes and a fractured right arm rendered him unable to administer his insulin. The physician felt the fall which resulted in the fracture was probably due to insulin reaction (convulsion). VNA service provided: 1. Administration of insulin every a.m. and p.m. 2. Observation for symptoms of diabetic hyper and hypo glycemia and reporting changes to physician. 3. Wrapping broken shoulder in position of abduction. 4. Observation and evaluation of self care of diabetes and equipment with appropriate guidance. 5. Range of motion exercise to right wrist, fingers, and flexion of right elbow. The alternative available would have been either hospitalization of a nursing home were it not for the VNA services. Cost of 33 days of Home Health Services (59 visits V.N.A.) $354.00 Cost of hospitalization in our area ($51. per day) $1,683.00 Cost of nursing home care ($17.50 per day) $577.50 Medicare has indicated it will pay for 16 of the 33 days of care after the $50.00 deductable has been met by the patient. The patient will be responsible for the additional 17 days of service but has indicated he is unable to pay this.
